The 3-Scan Patient Pathway: How Clinics Turn a £55 Early Scan into £280 in Pregnancy LTV

The short answer

A private baby scan clinic earns most of a patient’s value after her first scan. A £55 early scan followed by a gender scan and a 3D/4D scan comes to £280. In UpMedico’s working model, average revenue per patient is £87.10 without recall; consented reminders before each scan window lift repeat bookings and take it to £121.70. The £24 to £36 cost of acquiring her then falls from 28% to 41% of revenue to 20% to 30%.

What a one-scan patient costs the clinic

A £55 early scan is priced to win the click. On its own, it barely pays. In the ultrasound accounts UpMedico manages, a booked early pregnancy scan costs £24 to £36 to acquire, as published in the UK Private Ultrasound Clinic Growth Index.

Set that against the fee. Acquisition takes 44% to 65% of a £55 scan and leaves £19 to £31 before the sonographer, the room and the scanner hour are paid. The Growth Index treats around a third of the fee as healthy, and this is well past it.

The same patient looks different across a whole pregnancy. Hospitals in England offer at least two ultrasound scans, at 11 to 14 weeks and between 18 and 21 weeks, and a private clinic sells into the gaps around them: reassurance before the first NHS appointment, a gender scan, a 3D or 4D scan later on.

Price that journey at £55 for the early scan, £85 for a gender scan and £140 for a 3D/4D scan and it comes to £280, with every fee inside the ranges Which? published in December 2025. For a patient who books all three, the same £24 to £36 of acquisition is 9% to 13% of what she spends.

Most patients do not book all three. UpMedico’s working estimate, modelled on a cohort of 100 early-scan patients with no recall in place, has 18% returning for a gender scan and 12% for a 3D/4D scan. Average revenue per acquired patient stops at £87.10, and acquisition takes 28% to 41% of it.

That is £55, plus 18% of £85, plus 12% of £140. Across 100 early scans the clinic earns £8,710 from a pathway worth £28,000 if every patient completed it.

The first scan is the cost of entry. The pathway is the business.

Why generalist accounts count every mum as a new patient

Most Google Ads accounts are built to acquire. A conversion is a booking, every booking counts the same, and nothing in the report says whether the mum booking a gender scan was scanned at the same clinic eight weeks earlier.

If she comes back by searching the clinic’s name and clicking the brand advert at the top of the page, the clinic pays Google again for a patient it already had, and the report shows a cheap new conversion.

Google does offer a way to tell the two apart. Its customer lifecycle goals can bid higher for new customers than existing ones, and the re-engagement goal targets lapsed customers through Performance Max, using lists the advertiser uploads with Customer Match.

For a pregnancy scan clinic that route is closed. Google’s policy on health in personalized advertising treats health as a sensitive interest category, lists treatments for pregnancy among its examples and says advertiser-curated audiences such as Customer Match can’t be used when promoting in sensitive categories.

Both habits come from e-commerce templates, where a returning customer is simply retargeted with the next product. In pregnancy scanning the follow-up has to live in the clinic’s own booking system and messages, and an agency that only sees the ad account never looks at either.

How UpMedico builds repeat bookings into the three-scan pathway

UpMedico starts from the pregnancy, not the campaign. The first output is a pathway map: which scans the clinic sells, the gestational window and fee for each, and the value a patient has built by every step.

Paid campaigns then buy the first scan efficiently, and the rest of the pathway runs in the clinic’s own channels. Five steps.

1

Record the scan window at the first booking

The booking form asks for the estimated due date as an administrative field, so reception knows when the gender scan window opens, typically from 16 weeks, and when 3D/4D scans are usually carried out, between 26 and 32 weeks. The field sets the timing; clinical decisions stay with the clinician.

2

Ask for consent, separately for text and email

PECR allows a soft opt-in for a clinic’s own similar services, provided the patient can refuse when her details are collected and in every message after. Because the reminder is timed on pregnancy dates, we set it up on the patient’s explicit opt-in rather than on the soft opt-in alone.

3

One reminder ahead of each window

In UpMedico’s working model, reminders go out at around 14 weeks for the gender scan and 24 weeks for the 3D/4D scan, and 14% to 18% of messages turn into a booking. A six-year-old pregnancy scan clinic we spoke to this autumn wanted to add 10% off and worried it would look pushy. One timely message with a clear opt-out reads as service.

4

Stop rules before send rules

Every reminder is suppressed when an early scan shows a pregnancy that is not continuing, when the patient tells the clinic of a loss, or when she opts out. A gender scan offer sent to the wrong person does lasting harm, and no repeat booking rate justifies it.

5

Report returning patients on their own line

Recall bookings are tagged in the booking system and reported apart from paid acquisition, so a good reminder never makes a campaign look cheaper than it is. Cost per booked scan is then judged against the £280 pathway.

On the same 100-patient cohort, the reminders lift gender scan bookings from 18% to 34% and 3D/4D bookings from 12% to 27%. Average revenue per patient rises from £87.10 to £121.70, up 39.7%, with no additional ad spend.

The first scan still has to be bought well. The early pregnancy scan growth guide covers that campaign, Instant Booking vs. Enquiry Forms covers the booking route, and The Mid-Week Scanner Deficit explains why early pregnancy scans need their own budget cap. A returning patient who books a week ahead is also the easiest one to move into a quiet mid-week slot.

The acquisition side is the part UpMedico already runs for scan clinics. For London Private Ultrasound, a central London clinic offering abdominal, pelvic and pregnancy scans, the rebuilt account produced a 20% increase in total bookings against the previous quarter, with cost per lead down 41% after optimisation.

Pregnancy scan pathway benchmarks for UK clinics

The table combines the cost of acquiring a first scan in UpMedico-managed accounts with public price data and UpMedico’s working model of a 100-patient cohort. The pathway and cohort rows are modelled, not measured results, and are labelled that way.

Metric Value Source
Cost per booked scan, early pregnancy £24 to £36 UpMedico client data
Average early pregnancy scan fee £70 to £95 UpMedico client data
Public price ranges: early, gender, 4D £50 to £200; £50 to £190; £79 to £250 Which?, December 2025
Full pathway in the worked example £55 + £85 + £140 = £280 UpMedico working estimate
Gender and 3D/4D bookings, no recall 18% and 12% of patients UpMedico working estimate
Message-to-booking rate, reminders at 14 and 24 weeks 14% to 18% UpMedico working estimate
Gender and 3D/4D bookings, with recall 34% and 27% of patients UpMedico working estimate
Average revenue per patient, no recall £87.10 UpMedico working estimate
Average revenue per patient, with recall £121.70 (+39.7%) UpMedico working estimate
Acquisition as a share of revenue per patient 28% to 41% without recall; 20% to 30% with recall UpMedico working estimate
Acquisition as a share of the full £280 pathway 9% to 13% UpMedico working estimate

Read the two revenue-per-patient rows against the cost per booked scan before anything else. Without recall, acquisition takes 28% to 41% of what a patient spends, either side of the third the Growth Index treats as healthy; with recall it drops to 20% to 30% on the same ad spend.

A £55 entry price only makes sense for a clinic that runs recall. Without it, the clinic is better served pricing near the £70 to £95 average early pregnancy fee in our accounts, where the same cost per booked scan takes roughly 25% to 51% of the fee.

Where the second booking is won or lost on the phone

Returning patients do not all book online. In our call tracking data, 42% of early pregnancy scan bookings arrive by phone, and the mum ringing to ask whether 15 weeks is too soon for a gender scan is a booking reception either makes or loses.

The end of the first appointment is the other moment. A patient leaving her early scan reassured is the easiest one to book for the next window, if the person at the desk knows to offer it.

The Call-to-Booking Framework™

UpMedico’s Call-to-Booking Framework™ combines healthcare-compliant call tracking, call recording, and structured intake analysis to identify why patient calls fail to convert, improving call-to-booking conversion by up to 20% within 90 days.

The recordings show whether that happens. A caller told to ring back nearer the time has been handed to whichever clinic answers first in eight weeks, and the recording is usually the only place that loss shows up.

Recording patient calls means processing data concerning health, which UK GDPR treats as special category data, and details of a pregnancy fall within it. It is configured accordingly, with a consent message before the call connects, a defined retention period and access limited to the people who need it.

Practical wording for reception is in our free clinic phone script, and the commercial case for call tracking in clinics is set out separately.

Frequently asked questions

How much is a pregnancy scan patient worth to a private clinic?

More than her first scan, though less than the full pathway. A £55 early scan, an £85 gender scan and a £140 3D/4D scan come to £280. In UpMedico’s working model of 100 early-scan patients, average revenue per patient is £87.10 without recall and £121.70 with consented reminders, against a cost per booked early scan of £24 to £36.

Can a baby scan clinic text past patients about their next scan?

Yes, within PECR. A clinic can rely on the soft opt-in for its own similar services if the patient could refuse when her details were collected and can opt out in every message. Because the timing uses pregnancy dates, which are health data, an explicit opt-in at the first booking is the safer basis. Ask for text and email permission separately.

Can a pregnancy scan clinic use Customer Match to remarket to past patients?

Generally not. Google treats health as a sensitive interest category, names treatments for pregnancy among its examples, and does not allow advertiser-curated audiences such as Customer Match when promoting in sensitive categories. Retention for a scan clinic belongs in its own booking system, text messages and emails, measured separately from paid campaigns.

When should a scan clinic send the reminder for the next scan?

Ahead of each window rather than inside it. In UpMedico’s working model, reminders go out at around 14 weeks for a gender scan, typically offered from 16 weeks, and 24 weeks for a 3D/4D scan, usually carried out between 26 and 32 weeks. Every reminder stops if the patient reports a loss, if the early scan shows the pregnancy is not continuing, or if she opts out.
