The short answer
The best PPC agency for an ADHD or psychiatry clinic in the UK passes four tests. It works only with healthcare providers. It understands clinical intake, from triage calls to Right to Choose enquiries. It knows CQC rules, Google health policy and Irish HPRA limits. And it reports cost per qualified assessment rather than cost per lead. The comparison below scores each agency on those criteria using public evidence that any clinic director can check.

The generalist trap: why ADHD clinics outgrow ordinary PPC agencies
A generalist PPC agency is built to sell volume. That works for a gym or a conveyancing firm, where one enquiry is worth roughly the same as the next. In an ADHD clinic, most enquiries are not patients yet.
Across the UK accounts behind our UK Private ADHD Clinic Growth Index, 48% of form enquiries reach a triage call and 58% of those book, so fewer than three in ten become an assessment.
Triage is where the generalist loses the thread. Many enquirers in England are still weighing a funded assessment through Right to Choose, and a campaign that cannot separate them from self-paying adults fills the diary with callbacks. The agency counts a lead. The coordinator spends about 18 minutes on it and books nobody.
Copy is where the policy trouble starts. Generalist ads borrow what converts in other sectors: urgency, a promised outcome, sometimes a medication name in the headline. In a health category that copy meets Google’s healthcare and medicines policy before it earns a click, and repeated breaches can end in the kind of suspension we cover in our ADHD clinic compliance guide.
Reporting hides both problems. A dashboard of cheap leads can look healthy for months while the booked-assessment line stays flat, because nobody connected the click to the clinic’s booking system. A London clinic in our data pays about £340 per qualified assessment, and a cost-per-lead report cannot tell you whether you are near that figure or three times above it.
Seven questions to ask before you sign with an agency
Whether you are shortlisting an ADHD clinic marketing agency or a psychiatry PPC agency, a specialist answers each of these in a sentence. Send them in writing and compare the replies side by side.
Do you work only with healthcare providers?
A yes means your account sits beside other clinics that live under the same policies. A mixed client list means mental health is a side vertical, learnt on your budget.
Can you trace a booking in Cliniko or Pabau back to the keyword?
Cliniko warns that embedded booking widgets struggle to keep the traffic source; Pabau fires a booking-confirmed event for ad tracking. An agency that knows this has done the integration before.
What is your benchmark cost per qualified assessment?
A specialist gives a figure and the definition behind it. Ours is £340 in London and £275 in regional UK. An agency that replies with cost per lead is telling you what it measures.
What CQC experience do you have?
Listen for specifics. Your landing page must show your current CQC rating conspicuously under Regulation 20A, and the service you advertise has to match the regulated activity you are registered for.
Do you record and analyse intake calls?
In our call tracking data, 58% of London enquiries and 47% of regional ones arrive by phone. Without recordings, more than half of the funnel is invisible to the agency.
How do you filter out enquirers heading for Right to Choose?
The right answer involves the landing page, the form and the call script working together. A vague answer here means paying for clicks from people who were never going to pay.
How do you handle Google’s health restrictions, and Ireland?
Remarketing lists and Customer Match are off the table under the personalised advertising policy, and Irish campaigns need a separate build under HPRA rules.
Comparison matrix: UK Google Ads agencies for ADHD and psychiatry clinics
The matrix scores each agency on five criteria taken from the questions above. Every tick has to rest on something a clinic director can open and read: a service page, a published case study, a policy guide or a named client. UpMedico publishes this comparison and appears in it, so our own row is held to the same test.
A “Not found” does not prove an agency lacks the capability. It means we found no public evidence of it when we reviewed each site on 30 September 2026. For city-level shortlists, see our comparisons for ADHD and autism clinics in London and ADHD clinics in Dublin.

Red flags that should end the sales call
A guaranteed top position is the quickest tell. Google recalculates Ad Rank on every search from the bid, ad quality, thresholds and the context of the query, so no agency controls where your ad appears tomorrow morning. Google’s own guidance on organic results is blunter: no one can guarantee a #1 ranking.
One way to keep that promise is to bid heavily on your own clinic name, a search you would have won anyway.
Be wary of the agency that never asks how many assessments your clinicians can deliver each month. Paying for enquiries the clinic cannot see for months turns a marketing cost into a complaints problem, and a team with a full diary needs a very different campaign from one with empty Tuesday clinics.
Reports built on clicks and impressions alone are the floor, not a performance review. Google’s rules for third-party partners already require agencies to share cost, clicks and impressions every month. If that is all you receive, you are getting the policy minimum and paying a management fee for it. The line that matters connects spend to booked assessments.
Long contracts with no exit clause close the list. A twelve-month lock-in reassures the agency, not the clinic. Check who owns the Google Ads account before you sign. Google requires partners to give you your customer ID on request, and the account, its history and its conversion data should stay with you if you leave.
Question eight: how will you show me why my calls don’t convert?
Save this question for the end of the call, because it sorts agencies in a single reply. A generalist will talk about call extensions and cost per call. Both measure a phone ringing. Neither explains why a ringing phone did not become a booked assessment.
UpMedico’s Call-to-Booking Framework™ combines healthcare-compliant call tracking, call recording, and structured intake analysis to identify why patient calls fail to convert, improving call-to-booking conversion by up to 20% within 90 days.
In practice, each recorded call is scored against the clinic’s intake script: whether the caller heard the fee, and whether the coordinator offered a slot before the call ended. The keyword that produced the call sits beside the score, so a campaign change and a script change are judged on the same evidence.
An ADHD enquiry is health data. Recording it needs a lawful basis under UK GDPR Article 6 and a separate Article 9 condition for special category data, a clear notice at the start of the call, and a retention period agreed before the first recording exists. Recordings are then deleted on that schedule, not kept indefinitely.
What this looks like with UpMedico running your Google Ads
UpMedico manages Google Ads exclusively for private healthcare clinics in the UK and Ireland. Every engagement includes compliant campaign build, keyword-level call tracking and recording, and monthly analysis through our Call-to-Booking Framework™, so you see exactly which keywords produce booked assessments and scans, not just clicks.
If your current agency reports on clicks and impressions but can’t tell you your cost per booked patient, request a 20-minute Clinical Campaign Audit. We’ll show you the gap in your own account.

Is this comparison independent of UpMedico?
No, and it does not claim to be. UpMedico publishes the article and appears in the table. That is why each criterion is defined as a public test and the evidence column shows where every tick comes from. A clinic director can re-score any agency in the matrix, ours included, using only what each agency publishes.
What is a good cost per qualified assessment for an ADHD clinic in the UK?
Across the accounts behind UpMedico’s Growth Index, the cost per qualified assessment is about £340 in London and £275 in regional UK. The number only means something next to its definition: an enquiry that passed triage and booked, not a completed form. Any agency quoting a lower figure should explain what it counts as qualified.
Can an agency guarantee my clinic the top ad position on Google?
No. Google runs an auction on every search and recalculates Ad Rank each time from the bid, ad quality, thresholds, competition and the context of the query. Better ads and landing pages raise the odds of a top position, but nobody can promise it. A guarantee usually means the agency plans to bid very high on a narrow set of keywords.
What should a Google Ads agency report to an ADHD clinic every month?
Google’s third-party policy sets the floor: monthly cost, clicks and impressions at account level, with any management fee disclosed in writing before the first purchase and on every invoice. A clinic needs more than that, namely enquiries by channel, triage outcomes, booked assessments and cost per qualified assessment, with call recordings behind the phone figures.
